Clinical Outreach Navigator

Responsibilities

• Provide direct clinical support including harm reduction counseling and overdose mitigation supplies
• Deliver weekly clinical support to program staff and clients
• Consult with multidisciplinary teams to support clients with complex behavioral health, substance use, and medical needs
• Support housing placement, transition, and orientation processes
• Respond to crisis and high-risk situations in collaboration with leadership
• Establish and maintain relationships with outside agencies
• Meet with high-risk clients in community settings, treatment programs, hospitals, and houses of correction

Requirements

• Master's degree in a related field
• Experience working with adults experiencing severe mental illness or homelessness
• Ability to provide flexible, community-based services
• Valid driver's license and reliable transportation
