Clinical Outcomes Lead

Responsibilities

• Serve as the primary clinical partner for SNF customers, building relationships with Directors of Nursing and clinical leaders
• Define, measure, and communicate clinical impact, including hospital readmissions and staffing efficiency
• Develop clinical case studies and impact narratives to support customer retention and expansion
• Provide structured feedback to Product and Customer Success teams regarding clinical workflows and feature needs
• Monitor and interpret outcomes to develop SNF-specific dashboards and reporting frameworks

Requirements

• RN with 5+ years of experience, including direct SNF or post-acute care delivery
• 1–2 years in a clinical success, implementation, or advisory capacity
• Deep familiarity with CMS, PDPM, 5-Star Quality Rating System, and hospital readmission reporting
• Expertise in clinical data analysis and translating data into executive-level narratives
• Ability to work in technology-enabled care environments

Preferred Qualifications

• Experience working for a healthcare technology vendor or consultant
• Familiarity with leading SNF EMR platforms
• Proficiency with BI tools such as Tableau or Looker
• Experience with MDS assessment processes and clinical quality improvement initiatives
• Prior leadership experience in clinical operations
