About the role
Responsibilities
- Serve as the primary liaison between Ennoble Care and acute care facilities
- Educate patients and families on hospice and palliative care philosophy and services
- Provide emotional support and guidance during care transitions
- Collaborate with physicians and interdisciplinary teams to develop care plans
- Conduct nursing assessments and make clinical judgments
- Coordinate discharge planning with medical consultants
- Promote hospice and palliative care programs within hospitals and skilled nursing facilities
- Build relationships with facility leadership through education and outreach
Requirements
- Minimum of one year of experience in a comparable clinical setting
- Minimum of one year of hospice and/or marketer experience
- Experience in hospice and/or palliative care
- Graduate of an accredited nursing program (RN or LPN)
- Current nursing licensure as an RN or LPN in the applicable state
- Valid driver's license and access to a reliable vehicle

- BSN for RN candidates
