Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care through technology and clinical excellence.

Responsibilities

  • Serve as the daily on-site presence at assigned facilities to build relationships with case managers and discharge planners
  • Identify patients appropriate for home health services through proactive engagement
  • Conduct bedside visits with patients and families to explain services and answer questions
  • Ensure referral paperwork is complete, accurate, and submitted to the intake team in real time
  • Track pending referrals and follow up daily until admission and scheduling
  • Communicate patient outcomes and status updates back to referring facilities
  • Leverage clinical license to speak credibly with facility clinicians regarding care transitions
  • Enter referral information into EMR/intake systems accurately

Requirements

  • Active, unrestricted Texas license as an LVN/LPN, PTA, or COTA
  • Minimum 1 year of clinical experience in home health, hospital, SNF, or rehabilitation setting
  • Previous experience as a Clinical Liaison
  • Strong interpersonal and communication skills
  • Proficiency with EMR systems
  • Reliable transportation for travel between facilities

Preferred Qualifications

  • Familiarity with Medicare, Medicare Advantage, and commercial insurance requirements
  • Understanding of home health admission criteria and CMS Conditions of Participation
  • Experience with discharge planning workflows
  • Bilingual (English/Spanish)
