Abby Care is building the leading AI-native platform for family-led care. We combine clinical oversight with an AI-powered platform to train, enable, and support family caregivers in delivering high-quality care at home, helping health plans and government partners improve care outcomes.

Responsibilities

• Maintain a highly visible, daily presence within assigned hospitals to identify and map key decision-makers
• Conduct formal presentations and informal floor-level education to serve as a subject matter expert
• Actively prospect for eligible patients to drive and meet strict referral volume metrics
• Work closely with hospital staff and internal intake teams to ensure seamless patient transitions
• Utilize CRM to log facility activities, track relationship progress, and manage the pipeline

Requirements

• 4+ years of experience in a hospital or healthcare role
• Proven track record of building relationships with discharge planners, social workers, and nursing staff
• Experience working directly on hospital floors and in discharge planning departments
• Deep understanding of hospital workflows and patient transition complexities
