About the role
Responsibilities
- Coordinate the evaluation process for potential transplant candidates and manage the transplant waitlist.
- Provide direct patient care, including assessment of conditions, review of lab reports, and management of immunosuppression under supervision.
- Serve as a liaison between patients, physicians, clinical teams, and other healthcare providers to ensure seamless continuity of care.
- Ensure compliance with United Network for Organ Sharing (UNOS) policies and regulatory requirements.
- Facilitate clinical documentation, patient follow-up, and coordination of diagnostic testing and referrals.
- Participate in interdisciplinary rounds, administrative meetings, and patient education/support groups.
- Maintain professional competence through continuous education and adherence to safety regulations.
Requirements
- Bachelor’s Degree in Nursing (or equivalent in education and experience).
- Valid New York State RN License.
- 3 years of related clinical experience.
Preferred Qualifications
- Intermediate to advanced level MS Office skills.
