Clinical Coordinator

Responsibilities

• Serve as the central point of connection between providers, patients, caregivers, and healthcare partners
• Coordinate services and facilitate communication between care providers
• Manage patient needs and follow-up activities
• Help patients and families access necessary resources for health and well-being
• Ensure continuity of care and navigate complex healthcare needs

Requirements

• High school diploma or GED
• CENA certification, MA certification, or equivalent work experience
• Minimum 1 year of experience in medical field or medical office setting
• Strong grasp of medical terminology
• Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint)
• Reliable transportation with valid automobile insurance

Preferred Qualifications

• Associate's degree
• Experience with geriatric population and/or caregivers
• Previous experience in care coordination or medical office management
