Clinical Coordinator II - RN

Responsibilities

• Assess and collect patient health data and input medical information into the EMR.
• Develop care plans across the continuum, including inpatient, outpatient, and home care.
• Assist with medication management, renewals, and medical prior authorizations.
• Execute nursing interventions and administer immunizations and medications.
• Handle patient triage via telephone, MyChart, and in-person visits.
• Coordinate multidisciplinary healthcare services and arrange diagnostic tests.
• Review and act on laboratory and imaging test results.
• Manage communications from internal and external customers, including in-basket messages.
• Ensure infection control compliance within the practice.
• Precept new clinical staff, including Medical Assistants and junior nurses.

Requirements

• Bachelor’s degree in Nursing (or equivalent in education and experience).
• 2-4 years of related clinical experience.
• Current New York State RN license.
• BLS certification.
• Ability to perform duties involving exposure to blood, bodily fluids, and tissue.
• Ability to travel between clinical sites as required.
