About the role
Responsibilities
- Assess and collect patient health data and input medical information into the EMR.
- Develop care plans across the continuum, including inpatient, outpatient, and home care.
- Assist with medication management, renewals, and medical prior authorizations.
- Execute nursing interventions and administer immunizations and medications.
- Handle patient triage via telephone, MyChart, and in-person visits.
- Coordinate multidisciplinary healthcare services and arrange diagnostic tests.
- Review and act on laboratory and imaging test results.
- Manage communications from internal and external customers, including in-basket messages.
- Ensure infection control compliance within the practice.
- Precept new clinical staff, including Medical Assistants and junior nurses.
Requirements
- Bachelor’s degree in Nursing (or equivalent in education and experience).
- 2-4 years of related clinical experience.
- Current New York State RN license.
- BLS certification.
- Ability to perform duties involving exposure to blood, bodily fluids, and tissue.
- Ability to travel between clinical sites as required.
