Clinical Coordinator - Ambulatory Multi Organ Transplant

Responsibilities

• Manage clinical activities and coordinate patient care across the continuum to ensure continuity of care
• Perform direct patient care responsibilities and clinical duties
• Provide teaching and educational support to individuals and groups
• Participate actively in Quality Improvement initiatives
• Engage in academic and research activities and projects
• Handle administrative responsibilities and other cross-functional duties as assigned

Requirements

• Bachelor of Science Degree in Nursing
• Current licensure/certificate of competency from the College of Nurses of Ontario
• BCLS Level II Rescuer certification
• Up to 3 years of relevant experience, including experience in an advanced practice role
• Strong organizational, interpersonal, and communication skills
• Excellent decision-making, problem-solving, and time management abilities
• Ability to work both independently and collaboratively within a multi-disciplinary team
• Computer literacy and ability to work in a self-directed manner

Preferred Qualifications

• Completion of a recognized Master's degree in nursing
• Previous experience in Transplant nursing
• Experience in research activities
