Clinical Case Manager

Responsibilities

• Provide comprehensive psychological and behavioral case management services
• Divert individuals from the criminal justice system into care coordination
• Assess, monitor, and follow up on clients' ability to meet basic needs
• Conduct weekly outreach in jails, shelters, hospitals, and the community
• Assist clients with accessing food, shelter, healthcare, and public benefits
• Attend court with clients and coordinate legal system support
• Support daily living skills including budgeting, hygiene, and medication use
• Maintain all clinical documentation of consumer interactions
• Transport clients to various locations and agencies using a personal vehicle

Requirements

• Bachelor's degree (for CM I) or Master's degree (for CM II) in psychology, social work, or related human services
• Valid Colorado Driver's license with a recent clean driving record
• Ability to transport clients in a personal vehicle and obtain Business Use auto insurance
• Knowledge of trauma-informed principles and psychotherapy methods
• Familiarity with DSM-V and diagnostic techniques
• Understanding of HIPAA and confidentiality protocols

Preferred Qualifications

• One year of experience in mental health or human services
• Bilingual proficiency in Spanish or ASL
