Clinical Case Manager

Responsibilities

• Provide comprehensive psychological and behavioral case management services within a high-intensity treatment continuum
• Assist caseload in meeting basic needs including food, clothing, shelter, and medical/dental care
• Facilitate access to community resources and arrange or provide transportation for clients
• Educate and empower clients in daily living skills such as medication use, budgeting, and hygiene
• Support individuals in finding employment, vocational rehabilitation, or supportive housing
• Maintain clinical documentation of all consumer interactions
• Offer support and consultation to families, landlords, and community agencies to reduce stigma

Requirements

• Bachelor's degree (for CM I) or Master's degree (for CM II) in psychology, social work, or a related human services field
• Valid Colorado Driver’s license and a recent clean driving record
• Access to a safe, dependable vehicle for transporting clients
• Ability to obtain and carry Business Use auto insurance coverage
• Completion of Tuberculosis (TB) screening and testing prior to hire

Preferred Qualifications

• One year of experience in mental health or human services
• Bilingual proficiency in Spanish or ASL
