About the role
Responsibilities
- Provide comprehensive psychological and behavioral case management services within a high-intensity treatment continuum
- Assist caseload in meeting basic needs including food, clothing, shelter, and medical/dental care
- Facilitate access to community resources and arrange or provide transportation for clients
- Educate and empower clients in daily living skills such as medication use, budgeting, and hygiene
- Support individuals in finding employment, vocational rehabilitation, or supportive housing
- Maintain clinical documentation of all consumer interactions
- Offer support and consultation to families, landlords, and community agencies to reduce stigma
Requirements
- Bachelor's degree (for CM I) or Master's degree (for CM II) in psychology, social work, or a related human services field
- Valid Colorado Driver’s license and a recent clean driving record
- Access to a safe, dependable vehicle for transporting clients
- Ability to obtain and carry Business Use auto insurance coverage
- Completion of Tuberculosis (TB) screening and testing prior to hire
Preferred Qualifications
- One year of experience in mental health or human services
- Bilingual proficiency in Spanish or ASL
