Clinical Case Manager

Responsibilities

• Provide case management and clinical services to assigned participants in a residential and day program setting
• Support participants with financial information, benefits management, and supported employment
• Perform administrative duties related to case management
• Provide training and support to staff members in clinical and vocational areas
• Participate in the on-call rotation

Requirements

• Bachelor's degree in human services or a related field
• Four years of related experience or training working with the target client population

Preferred Qualifications

• Knowledge of Medicaid and benefits management
• Experience with supported employment activities
