Clinical Case Manager

Responsibilities

• Partner with children and families to provide mental health and behavioral support
• Teach parenting skills and developmental strategies
• Manage cases by attending treatment meetings and updating court and monthly reports
• Connect youth and families to essential community resources
• Enhance permanent connections for youth within the community
• Participate in weekly staff meetings and provide on-call support as part of a multi-disciplinary team
• Provide transportation for clients as needed

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field
• Experience working with youth and families
• Willingness to travel throughout the state

Preferred Qualifications

• Ability to work effectively as a team player
• Self-starter mindset with the ability to work independently
• Flexibility to manage personal schedules and goals
