About the role

Responsibilities

  • Provide case management services for adults in a residential Transitional Housing Program
  • Develop treatment plans and support individualized goal setting
  • Facilitate groups and coordinate care with internal and external providers
  • Assist residents with budgeting, benefits navigation, and community resource access
  • Teach daily living and independent living skills to support community transition

Requirements

  • Bachelor’s degree in psychology, Social Work, Human Services, or a related field
  • Experience in the mental health field preferred
  • Strong organizational, communication, and relationship-building skills
  • Ability to work collaboratively in a residential, team-based setting
