About the role
Responsibilities
- Provide case management services for adults in a residential Transitional Housing Program
- Develop treatment plans and support individualized goal setting
- Facilitate groups and coordinate care with internal and external providers
- Assist residents with budgeting, benefits navigation, and community resource access
- Teach daily living and independent living skills to support community transition
Requirements
- Bachelor’s degree in psychology, Social Work, Human Services, or a related field
- Experience in the mental health field preferred
- Strong organizational, communication, and relationship-building skills
- Ability to work collaboratively in a residential, team-based setting
