Clinical Case Manager III

Responsibilities

• Provide clinical and case management services to assigned participants, including individual and group therapy
• Conduct assessments, treatment planning, and crisis management
• Coordinate therapeutic services and manage benefits and financial information
• Provide team leadership for a multidisciplinary treatment team
• Offer clinical training and support for staff
• Coordinate IMR (Individual Placement and Support) activities where applicable

Requirements

• Master's Degree in Social Work or a related field
• Ability to work onsite in Nashua, NH
• Willingness to travel for day trips, conferences, training, and meetings

Preferred Qualifications

• Licensure in the social work or mental health field (or eligibility for licensure)
• Experience working with benefits and IMR/supported employment
• Experience working with adults living with Serious Mental Illness (SMI) or Serious and Persistent Mental Illness (SPMI)
