Clinical Care Manager

Responsibilities

• Lead and support a multidisciplinary clinical team to deliver outstanding resident care
• Oversee resident assessments, care planning, clinical reviews, and complex care management
• Drive compliance with Aged Care Quality Standards, clinical governance, and best-practice care
• Mentor, coach, and develop clinical staff through visible leadership and performance support
• Monitor clinical outcomes, incidents, and quality indicators to promote continuous improvement
• Provide oversight of medication management, infection prevention, and resident safety initiatives
• Collaborate with families, medical practitioners, and allied health professionals to support resident outcomes
• Contribute to workforce planning, rostering, recruitment, and staff development

Requirements

• Current registration as a Registered Nurse with AHPRA
• Relevant tertiary qualifications in Nursing or a related discipline
• Minimum 5 years of clinical experience within aged care, health, disability, or community services
• Previous leadership or supervisory experience within a clinical environment
• Demonstrated experience managing complex clinical presentations and high-risk care situations
• Strong understanding of Aged Care Quality Standards and clinical governance
• NDIS clearance
