Clinical Care Manager Assistant - LPN

Responsibilities

• Support the Clinical Care Manager and nursing team to ensure smooth team functioning
• Manage patient documentation and timely data entry into the EMR system
• Facilitate interdisciplinary group (IDG) meetings by preparing agendas, sign-in sheets, and care plan templates
• Maintain accurate spreadsheets for team census, disaster preparedness, and oxygen vendor usage
• Notify team members, physicians, and outside vendors regarding patient status changes or deaths
• Perform administrative tasks including running productivity reports, auditing nursing deficiencies, and managing office supplies
• Act as a professional liaison between all team disciplines and provide emotional support to patients and families

Requirements

• Current Florida license as a Licensed Practical Nurse (LPN)
• Minimum of one year of nursing experience
• Proficiency in Electronic Medical Records (EMR) software
• Strong ability to multi-task and adapt to changing clinical procedures
