Clinical Care Manager Assistant - LPN

Responsibilities

• Support the Clinical Care Manager and nursing team to ensure smooth operational functioning
• Manage patient documentation and data entry within the Electronic Medical Record (EMR) system
• Facilitate interdisciplinary group (IDG) meetings by preparing agendas, sign-in sheets, and care plan templates
• Monitor and update patient status changes, census spreadsheets, and recertification alerts
• Coordinate notifications to team members, physicians, and outside vendors regarding patient status or orders
• Perform administrative tasks including running productivity reports, auditing nursing deficiencies, and managing office supplies
• Maintain professional communication and provide emotional support to patients and their families in accordance with hospice philosophy

Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Florida
• Minimum of one (1) year of nursing experience
• Proficiency in Electronic Medical Records (EMR) software
• Strong ability to multi-task and adapt to changing clinical procedures
• Excellent organizational and communication skills
