About the role

Responsibilities

  • Coordinate care for patients undergoing interventional nephrology procedures, including dialysis access creation and maintenance
  • Facilitate referrals for renal biopsies, tunneled catheters, and other vascular access devices
  • Perform clinical assessments and telephone triage for patients with access-related concerns
  • Educate patients and families regarding procedures, vascular access care, and treatment plans
  • Collaborate with nephrologists, radiology staff, and dialysis centers to ensure seamless care transitions
  • Maintain referral databases and contribute to quality assurance programs following KDOQI guidelines
  • Document all patient interactions, assessments, and care plans within the electronic medical record

Requirements

  • Current Registered Nurse (RN) licensure in the State of Michigan
  • BSN or an Associate/Diploma in Nursing combined with an MSN
  • Minimum of 5 years of hemodialysis and ESRD nursing experience in acute or chronic settings
  • 2 years of recent experience in a clinical leadership role (e.g., charge nurse or preceptor)
  • Valid ACLS, BLS, and CAC Certifications

Preferred Qualifications

  • Previous experience as a clinical care coordinator
  • Experience in adult ambulatory care and telephone triage
  • Background in case management
  • Proficiency with electronic medical record systems and clinical documentation
