Clinical Care Coordinator

Responsibilities

• Serve as a clinical resource and consultant specializing in wound and ostomy care and pressure ulcer prevention for pediatric and adult populations
• Collaborate with patients, families, and medical staff to develop and implement care recommendations for vascular wounds, diabetic ulcers, and surgical wounds
• Develop patient, family, and nursing teaching materials regarding wound and ostomy management
• Participate in the development of institutional policies, procedures, and guidelines based on evidence-based practice
• Plan and implement nursing educational programs, including staff development and central orientation
• Review institutional pressure ulcer data to identify improvement opportunities and quality initiatives
• Consult on Negative Pressure Wound Therapy and provide recommendations for specialized wound and ostomy supplies and equipment
• Oversee patient evaluation and provide consultation for therapeutic surfaces and specialty beds

Requirements

• Current RN licensure to practice nursing in the State of Michigan
• Bachelor's degree in Nursing OR an Associate degree/diploma in Nursing with a Master's degree in Nursing
• Minimum of two (2) years of recent wound or ostomy direct patient care experience in an acute care setting
• WOCN certification

Preferred Qualifications

• Prior experience specifically as a wound ostomy nurse
• Previous experience working with both adult and pediatric populations
