About the role
Responsibilities
- Provide expert care coordination, consultation, education, and advocacy for patients and families within the Pediatric Feeding Clinic.
- Perform focused nursing assessments to guide clinical decision-making and patient care.
- Manage patient triage and follow-up via telephone, portal messages, and encounter follow-up.
- Support clinic workflows including patient intake, medication reconciliation, and nurse visits.
- Educate patients and families on diagnoses, medications, and individualized care plans.
- Collaborate with the primary medical team, case managers, and social workers to implement organized care plans for complex patients.
- Coordinate transitions of care between inpatient and outpatient settings.
- Document all nursing assessments, education, and communications accurately within the EHR.
Requirements
- Bachelor's degree in Nursing OR an Associate's degree in Nursing with a Master's degree.
- Current Michigan RN Licensure.
- Minimum of 3 years of recent inpatient experience with neonates/infants in a NICU or a general pediatric floor.
- Minimum of 1 year of experience with neonates/infants with complex feeding difficulties.
- Minimum of 1 year of experience physically managing NG and G tubes (placement, assessment, or caregiver education).
- Proficiency with electronic health record documentation, specifically Epic.
Preferred Qualifications
- Experience working specifically with neonates/infants with feeding difficulties.
- Minimum of 1 year of experience in clinical care coordination or case management.
- Minimum of 1 year of experience in program development.
- Minimum of 1 year of RN experience with phone triage in an ambulatory care setting.
- Certification in Ambulatory Nursing.
