Clinic Patient Care Navigator

Responsibilities

• Serve as the first point of contact for inbound patient calls and handle appointment scheduling, rescheduling, and confirmation
• Act as the on-site Rely Health liaison to support provider teams and improve technology workflows
• Assist patients with medical and social coordination, including pharmacy linkages and referrals to social workers
• Use motivational interviewing to communicate with patients in a respectful and culturally appropriate manner
• Identify care gaps through data analysis and initiate proactive, metric-driven outreach
• Provide constructive product feedback to improve the technology stack and front-desk experience
• Maintain accurate documentation in all required systems and complete reporting on time

Requirements

• High school diploma or GED
• 2+ years of experience in patient care navigation or healthcare coordination
• Proven customer service and healthcare experience
• Familiarity with healthcare technology platforms and tools
• Understanding of HIPAA and confidentiality requirements
• Ability to work independently in a fast-paced environment

Preferred Qualifications

• Experience working with underserved populations or in public health settings
• Knowledge of Medicare, Medicaid, and commercial insurance policies
• Proficiency in EHR/EMR documentation
• 40+ wpm typing proficiency
• Bilingual proficiency
