About the role
Responsibilities
- Fulfill daily routines and fundamental care requirements for patients, including meal assistance, hygiene, grooming, and ambulation
- Execute individualized care plans addressing diet restrictions, safe transferring, and fall prevention
- Monitor and report changes in patient condition, such as skin integrity, mental status, and nutritional factors, to the Registered Nurse
- Document care and patient changes accurately using hospital-approved progress notes and summaries
- Operate medical equipment such as lifts and wheelchairs safely
- Assist with psychosocial patient needs and respond to call bells to ensure a safe and supportive environment
Requirements
- Valid registration with the College of Nurses of Ontario
- Current registration with the Health and Supportive Care Providers Oversight Authority (HSCPOA)
- Personal Support Worker (PSW) Certification from an accredited program
- High school diploma (Grade 12) or equivalent
- Proficiency in Microsoft Office (Word), Internet usage, and Outlook
- Strong verbal and written communication skills
Preferred Qualifications
- One year of full-time equivalent work experience within the past three years
- Basic Cardiac Life Support (BCLS) certificate
