About the role
Responsibilities
- Support patients' daily routines including meal assistance, hygiene, grooming, and toileting
- Execute individualized care plans addressing diet restrictions, safe transferring, and fall prevention
- Monitor and report changes in patient condition, such as skin integrity, mental status, and nutritional factors, to the Registered Nurse
- Assist with psychosocial patient needs and respond to call bells promptly
- Document care and patient changes accurately using hospital-approved systems
- Operate medical equipment such as lifts and wheelchairs safely
- Maintain a safe environment by adhering to safety standards and using personal protective equipment
Requirements
- Valid registration with the College of Nurses of Ontario
- High school diploma (Grade 12) or equivalent
- Current registration with the Health and Supportive Care Providers Oversight Authority (HSCPOA)
- Personal Support Worker (PSW) Certification from an accredited program
- Proficiency in Microsoft Office (Word), Internet usage, and Outlook
- Strong verbal and written communication skills
- Ability to work collaboratively within an interprofessional team
Preferred Qualifications
- At least one year of full-time equivalent work experience within the past three years
- Basic Cardiac Life Support (BCLS) certificate
