CHC Service Coordinator

Responsibilities

• Manage an active caseload of seniors and individuals with disabilities, ensuring all state-mandated ratios and regulatory deadlines are met.
• Conduct in-home assessments and lead the Person-Centered Service Planning (PCSP) process to support independent living.
• Coordinate access to Long-Term Services and Supports (LTSS), medical assistance, and community resources including housing and social services.
• Monitor participant progress and collect program data to ensure compliance with OPS reporting and regulatory requirements.
• Build collaborative relationships with internal teams and partner providers to facilitate seamless physical and behavioral care delivery.
• Educate members on their rights, service alternatives, and self-direction opportunities.

Requirements

• Background in case management or care coordination with experience conducting in-home assessments.
• Ability to work in the field at least 75% of the time.
• Valid driver's license and reliable transportation for daily community travel.
• Bachelor's degree in social work, psychology, or a related field preferred, OR at least 3 years of experience in a social service or healthcare setting.
• Strong knowledge of HIPAA compliance and ability to maintain strict confidentiality.
• Ability to work independently in a hybrid/virtual setting with a private, distraction-free workspace.
• Technical capability to meet specific internet speed requirements (min 20Mb/s download, 5Mb/s upload).

Preferred Qualifications

• Experience working specifically with seniors or individuals with disabilities requiring LTSS.
• Knowledge of the home and community-based service system.
• Demonstrated cultural competency and sensitivity to diverse backgrounds.
