About the role
Responsibilities
- Provide personal care for hospice patients under the direction of a licensed clinician
- Follow established Plans of Care and adhere to standard precautions
- Document all care provided in a timely and accurate manner
- Observe and report any changes in patient status to clinical staff
- Maintain strict patient, family, and caregiver confidentiality
- Deliver duties with a high level of professionalism and excellent customer service
Requirements
- High School Diploma or equivalent
- Current Massachusetts CNA certification
- Experience with the end-of-life care process
- Ability to observe and communicate changes in patient status
- Successful completion of required training programs and annual in-service requirements
