About the role
Responsibilities
- Provide direct patient care in home settings, assisted living facilities, or nursing homes under the direction of a licensed clinician.
- Perform personal care tasks including hygiene, bathing, shaving, and emotional support.
- Monitor and record vital signs, skin condition, and other patient data as outlined in the care plan.
- Assist with activities of daily living, including feeding, transfers (using equipment like Hoyer lifts), and ambulation.
- Perform basic wound care and catheter maintenance as directed by the RN case manager.
- Assist with prescribed exercises and range of motion (ROM) activities.
- Provide verbal cues and prompts for oral medication adherence.
- Manage patient caseload and scheduling, including navigating routes to service addresses.
- Document all clinical care according to agency policy and regulatory standards.
Requirements
- High School diploma or GED.
- Certified Nurse Aide (CNA) certification in the state of New Mexico.
- Current BLS Certification required at the time of hire.
- Valid driver's license and access to a reliable vehicle.
- Proof of auto insurance and a good driving record.
Preferred Qualifications
- One year of direct patient care experience in a clinical setting, home health, or hospice environment.
