About the role

Responsibilities

  • Provide integrated case management services through the Pathway of Hope initiative, supporting participants in reaching goals and building hope.
  • Perform all aspects of the case management process, including selection, intake, assessment, goal planning, action, transition, and follow-up.
  • Meet with participants in person, via video conferencing, or in community settings such as homes or public spaces.
  • Collaborate with interdisciplinary teams, including spiritual care representatives and volunteers, to provide holistic support.
  • Maintain accurate and timely records, including detailed case notes and service statistics.
  • Network with community agencies and service providers to facilitate referrals and access to resources.
  • Support ministry unit integration by promoting opportunities for volunteers and corps members to engage with families.

Requirements

  • 2-year Community College Diploma in a human services field (e.g., Social Services Worker).
  • Minimum of 2 years of related casework experience, including documentation and community outreach.
  • Valid driver's licence and access to a reliable vehicle for travel between service locations.
  • Proficiency in Microsoft Office and ability to perform accurate data entry.
  • Ability to undergo a vulnerable sector screening and background checks.
  • Strong interpersonal, written, and verbal communication skills.

Preferred Qualifications

  • University degree in a relevant field, such as a Bachelor of Social Work or a Bachelor of Arts in Social Services.
  • Experience in community development or program facilitation.
  • Certifications in First Aid/CPR and Non-Violent Crisis Intervention.
