About the role
Responsibilities
- Provide integrated case management services through the Pathway of Hope initiative, supporting participants in reaching goals and building hope.
- Perform all aspects of the case management process, including selection, intake, assessment, goal planning, action, transition, and follow-up.
- Meet with participants in person, via video conferencing, or in community settings such as homes or public spaces.
- Collaborate with interdisciplinary teams, including spiritual care representatives and volunteers, to provide holistic support.
- Maintain accurate and timely records, including detailed case notes and service statistics.
- Network with community agencies and service providers to facilitate referrals and access to resources.
- Support ministry unit integration by promoting opportunities for volunteers and corps members to engage with families.
Requirements
- 2-year Community College Diploma in a human services field (e.g., Social Services Worker).
- Minimum of 2 years of related casework experience, including documentation and community outreach.
- Valid driver's licence and access to a reliable vehicle for travel between service locations.
- Proficiency in Microsoft Office and ability to perform accurate data entry.
- Ability to undergo a vulnerable sector screening and background checks.
- Strong interpersonal, written, and verbal communication skills.
Preferred Qualifications
- University degree in a relevant field, such as a Bachelor of Social Work or a Bachelor of Arts in Social Services.
- Experience in community development or program facilitation.
- Certifications in First Aid/CPR and Non-Violent Crisis Intervention.
