Case Manager/QIDP

Responsibilities

• Support individuals with intellectual disabilities in 24hr Supportive Living Arrangements
• Develop personal goals, action plans, and service plans for individuals
• Assist individuals in obtaining community resources, housing, and benefits
• Conduct recruitment tasks including interviewing and reference checks
• Support staff scheduling and conduct training on ID/DD, HIPAA, and incident reporting
• Perform home visits to ensure quality assurance and safe living environments
• Coordinate and attend PCP and quarterly support team meetings
• Complete required paperwork for case management billing

Requirements

• At least one year of experience working directly with people with intellectual or developmental disabilities
• Bachelor's degree in a human services field (e.g., Social Work, Psychology, Occupational Therapy, etc.) or a medical credential (RN, MD, DO)
• 1-2 years of experience serving as a QIDP or equivalent
• Proficiency with Microsoft Office, Google Workspace, or CRMs
• First Aid and CPR certification
• Ability to handle confidential information securely
• Reliable transportation for field visits and home coverage

Preferred Qualifications

• Experience working within interdisciplinary teams
• Familiarity with case management software
• Continuing education in behavioral plan implementation
