Case Manager

Responsibilities

• Plan, monitor, and coordinate patient care using established clinical pathways
• Identify patient and family needs for multidisciplinary specialist consultation
• Conduct or participate in patient care conferences
• Assess discharge needs and ensure timely completion of all discharge-related functions
• Provide consultation to staff nurses regarding patient status and care planning
• Document comprehensive information regarding the patient's hospital course in the medical record

Requirements

• Bachelor's degree in nursing from an NLN accredited school of nursing
• Current Illinois Registered Nurse license
• 3 or more years of nursing experience
• Proven experience working in case management or care coordination
• If holding an Associate Degree in Nursing, must enroll in a BSN program within 6-9 months of hire

Preferred Qualifications

• Certification in Case Management (e.g., CCM, CCCTM)
• BSN degree or higher
