Case Manager

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services to help them reach those goals
• Complete assessments, crisis plans, and individualized service plans
• Work collaboratively with the interdisciplinary clinical team to coordinate care with families, medical providers, and community resources
• Conduct outreach and follow-up for services, including crisis interactions, missed appointments, and hospital discharges
• Perform all case management functions for assigned caseloads, including identifying needs for specialty evaluations
• Pursue best practice outcomes for individuals with mental illness, including support for employment, housing, and community tenure

Requirements

• GED or High School diploma with at least 1.5 years of behavioral health experience OR an Associate degree in a medical/behavioral field OR a Bachelor's degree in behavioral health
• Valid Arizona driver’s license, minimum age of 21, and at least 4 years of driving experience
• Ability to obtain an Arizona Fingerprint Clearance card (Level 1) within 7 working days of employment
• Proficiency in Microsoft Word, Excel, and Outlook
• Minimum typing speed of 25 wpm
• Ability to pass a background check, TB test, and other pre-employment screenings

Preferred Qualifications

• Bachelor's degree in a non-behavioral field with 1 year of behavioral health experience
• Master's degree or higher in the field of behavioral health
• Certified Case Manager credential
• Basic understanding of the DSM-V
• Experience with electronic medical records, specifically NextGen
