Case Manager-Social Services

Responsibilities

• Assess patient needs within a defined geographical scope to determine case management requirements and interventions
• Coordinate care through the continuum by collaborating with multidisciplinary teams, physicians, and families
• Facilitate timely and safe discharges by managing barriers and developing individualized plans of care
• Assist the hospital in meeting budgeted length of stay (LOS) through active management of avoidable days
• Collaborate with Utilization Review regarding insurance verification, benefits, and managing concurrent denials
• Monitor readmission issues and escalate concerns to the healthcare team
• Ensure compliance with regulatory agencies including CMS, The Joint Commission, and the Department of Public Health
• Educate patients and families regarding medical recommendations, discharge plans, and payer benefits

Requirements

• Graduate of an accredited professional nursing school
• 3+ years of acute care nursing experience, including inpatient and outpatient settings
• Current Registered Nurse (RN) license in the state of Illinois

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Experience in Case Management, Utilization Management, or Discharge Planning
