About the role

Responsibilities

  • Facilitate the planning process and develop the Individual Program Plan (IPP) or Individual Family Service Plan (IFSP)
  • Coordinate and support the implementation of the Consumers plan
  • Advocate on behalf of consumers to protect their rights under the law
  • Assist the consumer and family in obtaining essential diagnosis and evaluation
  • Monitor the consumer’s health and safety
  • Maintain all documentation and contacts as required by law, regulation and policy
  • Complete functional and/or developmental reviews in a timely manner

Requirements

  • Case Manager I: BA in Social Work, Psychology, or related field plus 1 year experience, OR BA in unrelated human services field plus 4 years experience with developmentally disabled
  • Case Manager II: Master’s Degree in Social Work, Psychology, or related human services field
  • Ability to communicate clearly and effectively, verbally and in writing
  • Automobile and valid driver's license required
  • Cultural awareness and sensitivity to work with diverse populations

Preferred Qualifications

  • Spanish fluency
