About the role
Responsibilities
- Conduct client assessments, screenings, and intake interviews to develop individualized service plans.
- Perform regular in-person, phone, and virtual case management visits to monitor client progress.
- Connect clients to community resources, healthcare providers, housing agencies, and workforce programs.
- Advocate for clients to ensure access to appropriate services and support systems.
- Maintain accurate and timely case notes, assessments, and program documentation in compliance with regulatory requirements.
- Provide crisis intervention and problem-solving support to address barriers to stability.
- Manage data entry within case management systems and assist with program reporting and audits.
Requirements
- High school diploma or GED.
- At least 7 years of case management experience.
- Valid driver's license and reliable transportation for local travel and client visits.
- Proficiency in Microsoft Office and basic computer skills.
- Ability to work in various settings, including office, field, and community-based environments.
Preferred Qualifications
- Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
- Experience working with vulnerable populations, including individuals with disabilities or those experiencing homelessness.
- Knowledge of housing stabilization, veteran services, or Medicaid programs.
- Experience using HMIS, CaseWorthy, ClientTrack, or similar case management software.
- Bilingual abilities.
