About the role

Responsibilities

  • Conduct client assessments, screenings, and intake interviews to develop individualized service plans.
  • Perform regular in-person, phone, and virtual case management visits to monitor client progress.
  • Connect clients to community resources, healthcare providers, housing agencies, and workforce programs.
  • Advocate for clients to ensure access to appropriate services and support systems.
  • Maintain accurate and timely case notes, assessments, and program documentation in compliance with regulatory requirements.
  • Provide crisis intervention and problem-solving support to address barriers to stability.
  • Manage data entry within case management systems and assist with program reporting and audits.

Requirements

  • High school diploma or GED.
  • At least 7 years of case management experience.
  • Valid driver's license and reliable transportation for local travel and client visits.
  • Proficiency in Microsoft Office and basic computer skills.
  • Ability to work in various settings, including office, field, and community-based environments.

Preferred Qualifications

  • Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
  • Experience working with vulnerable populations, including individuals with disabilities or those experiencing homelessness.
  • Knowledge of housing stabilization, veteran services, or Medicaid programs.
  • Experience using HMIS, CaseWorthy, ClientTrack, or similar case management software.
  • Bilingual abilities.
