Case Manager of Justice Related Services

Responsibilities

• Engage adults with serious mental illness to facilitate support and linkage to resources for successful reentry, including housing, career, and community involvement
• Meet with clients in various settings including jails, the community, courts, hospitals, and through law enforcement
• Organize and coordinate services for adults in the forensic system using Blended Case Management guidelines
• Ensure the Access Services philosophy of care is implemented to meet individual client needs
• Build collaboration through community outreach and awareness
• Complete all required case notes, documentation, and attend regular team meetings and case reviews

Requirements

• Bachelor's degree in psychology, social work, criminal justice, or a related field
• Valid driver's license with 2+ years of driving experience
• Reliable vehicle with adequate insurance and willingness to travel throughout Montgomery County
• Ability to work independently and prioritize tasks while effectively collaborating with a team
• Knowledge of the criminal justice system and serious mental illness

Preferred Qualifications

• 2+ years of case management experience, preferably with adults with serious mental illness
• Previous experience related to the criminal justice system
