About the role
Responsibilities
- Receive and respond to resident referrals, conducting assessments using the Risk Assessment Management Framework
- Develop holistic, strengths-based, and culturally appropriate case management plans using SMART goals
- Coordinate referrals to relevant support services and facilitate case conferences with stakeholders
- Build respectful, professional relationships with residents and maintain strong boundaries
- Respond to complex cases and emergencies, supporting individuals and families through crisis and transitions
- Maintain accurate, up-to-date records and documentation in line with organizational policies
- Demonstrate child-safe practice and adhere to all relevant legislation and proactive risk reporting
Requirements
- Two years of experience in Human Services, or a relevant degree with substantial equivalent experience
- Demonstrated experience applying the Risk Assessment Management Framework in a support role
- Cultural competency and experience working respectfully with Aboriginal and Torres Strait Islander peoples
- Knowledge of issues contributing to homelessness, such as family violence, mental ill health, and substance use
- Strong communication, engagement, and time management skills
- IT proficiency, including Microsoft Office and case note writing
- Full driver licence with no restrictions
- Ability to pass a national police history check and obtain a clearance to work with children/vulnerable people
