About the role

Responsibilities

  • Receive and respond to resident referrals, conducting assessments using the Risk Assessment Management Framework
  • Develop holistic, strengths-based, and culturally appropriate case management plans using SMART goals
  • Coordinate referrals to relevant support services and facilitate case conferences with stakeholders
  • Build respectful, professional relationships with residents and maintain strong boundaries
  • Respond to complex cases and emergencies, supporting individuals and families through crisis and transitions
  • Maintain accurate, up-to-date records and documentation in line with organizational policies
  • Demonstrate child-safe practice and adhere to all relevant legislation and proactive risk reporting

Requirements

  • Two years of experience in Human Services, or a relevant degree with substantial equivalent experience
  • Demonstrated experience applying the Risk Assessment Management Framework in a support role
  • Cultural competency and experience working respectfully with Aboriginal and Torres Strait Islander peoples
  • Knowledge of issues contributing to homelessness, such as family violence, mental ill health, and substance use
  • Strong communication, engagement, and time management skills
  • IT proficiency, including Microsoft Office and case note writing
  • Full driver licence with no restrictions
  • Ability to pass a national police history check and obtain a clearance to work with children/vulnerable people
