Case Manager

Responsibilities

• Conduct intake assessments, including pre-tenancy interviews, and manage housing and tenancy waitlists
• Deliver tailored, trauma-informed case management and develop person-centred plans to support client goals
• Provide wraparound support through advocacy, referrals, and coordination with internal and external services
• Build strong, culturally safe relationships with clients to understand and respond to their immediate and long-term needs
• Work collaboratively with the Department of Housing and stakeholders to secure and sustain tenancy outcomes
• Support tenancy establishment and maintenance, including move-ins and accessing essential services
• Maintain accurate and timely case notes, reports, and documentation for program compliance

Requirements

• Certificate IV or degree in community/human services with demonstrated case management experience
• Experience working with diverse communities, including Aboriginal and Torres Strait Islander peoples
• Sound understanding of homelessness, housing instability, family violence, mental health, and substance use
• Demonstrated ability to deliver trauma-informed care and manage complex client needs
• Excellent communication, advocacy, and stakeholder engagement skills
• Strong organisational and time management skills, with the ability to work in a remote setting
• Full driver's licence with no restrictions
