About the role
Responsibilities
- Deliver comprehensive, person-centred case management to young people aged 12–25
- Conduct intake, assessments, and develop tailored case plans with targeted intervention strategies
- Build strong, trusting relationships with clients to encourage engagement and participation
- Provide advocacy and coordinate referrals by working with internal teams and external stakeholders
- Deliver outreach support across the Pilbara region, including transporting clients to appointments
- Maintain accurate administrative records, including case notes, reports, and data reporting
- Facilitate capacity building and life skills development for clients
Requirements
- Demonstrated experience in the AOD, community services, or human services sector
- Minimum Cert IV in AOD or a relevant degree in psychology, social work, or human services
- Strong understanding of Aboriginal cultural systems and protocols to deliver culturally safe practice
- Proven ability to engage young people using a trauma-informed, person-centred approach
- Ability to manage competing priorities and work autonomously in a regional context
- Full driver licence with no restrictions
