Case Manager

Responsibilities

• Deliver comprehensive, person-centred case management to young people aged 12–25
• Conduct intake, assessments, and develop tailored case plans with targeted intervention strategies
• Build strong, trusting relationships with clients to encourage engagement and participation
• Provide advocacy and coordinate referrals by working with internal teams and external stakeholders
• Deliver outreach support across the Pilbara region, including transporting clients to appointments
• Maintain accurate administrative records, including case notes, reports, and data reporting
• Facilitate capacity building and life skills development for clients

Requirements

• Demonstrated experience in the AOD, community services, or human services sector
• Minimum Cert IV in AOD or a relevant degree in psychology, social work, or human services
• Strong understanding of Aboriginal cultural systems and protocols to deliver culturally safe practice
• Proven ability to engage young people using a trauma-informed, person-centred approach
• Ability to manage competing priorities and work autonomously in a regional context
• Full driver licence with no restrictions
