Case Manager

Responsibilities

• Deliver intensive, strengths-based case management to families with complex needs
• Conduct home visits and provide outreach-based support to engage families in their communities
• Undertake comprehensive assessments and develop tailored case plans to ensure child safety
• Support parents to strengthen parenting capacity and improve family functioning
• Identify and respond to risks relating to child safety and wellbeing
• Coordinate with internal and external services to deliver integrated support
• Maintain accurate case notes, reports, and compliance documentation

Requirements

• Qualification in Social Work, Psychology, Welfare, Community Services, or a similar field
• Demonstrated experience in case management, specifically working with families and children
• Knowledge of domestic violence, mental health, and child development
• Ability to engage hard-to-reach families and build trust
• Current driver's licence and willingness to travel
• Current First Aid Certificate
• Ability to pass a national police history check and obtain a clearance to work with children
