Case Manager

Responsibilities

• Provide case management to children and young people in various out-of-home care arrangements, primarily foster care
• Build positive relationships with children, young people, and carers through regular home and outreach visits
• Ensure the client's voice is included in care planning and support their transition when leaving care
• Coordinate supports and services within a care team to meet the health, development, and wellbeing needs of clients
• Provide regular support and coaching to foster carers to ensure a safe and secure environment
• Deliver psychological assessment, therapy, and behavior support to address trauma and developmental needs

Requirements

• Bachelor's degree in Social Work, Social Science, Psychology, or Human Services
• Experience in child protection case management or providing case management to children and young people
• Strong knowledge of trauma-informed care and responses to pain-based behaviors
• Demonstrated ability to manage complex caseloads and prioritize competing demands
• Experience developing and implementing quality case plans or individual plans
• Current unrestricted driver’s licence
• Ability to clear probity checks, including a National Criminal History Record Check and Working with Children Check
