About the role
Responsibilities
- Provide intensive case management and housing stabilization services to homeless Veteran families
- Coordinate client intake, conduct initial assessments, and maintain electronic client files
- Perform street and venue-based outreach to homeless Veterans
- Develop individual budgets and service plans, involving clients in the design of their own supportive services
- Provide service coordination and linkage to VA services, mental health, substance abuse treatment, and vocational assistance
- Conduct home visits and weekly meetings to identify short and long-term goals
- Maintain accurate HMIS database records and prepare programmatic reports
- Collaborate with Housing Specialists to maintain positive relationships with landlords
- Provide crisis intervention as needed to ensure client safety and stability
Requirements
- High school diploma required (Bachelor’s degree in Social Services or related field preferred)
- 1 year of direct support experience with individuals experiencing homelessness, mental health, or substance abuse issues
- Valid California driver license and a reliable personal vehicle
- Proficiency in Microsoft Office suite (Word, Excel, Outlook, and Teams)
- Ability to maintain a calm demeanor in stressful situations and provide excellent customer service
- Ability to pass criminal records clearance
Preferred Qualifications
- Experience with the Homeless Management Information System (HMIS)
- Ability to complete CPR/First Aid training
