Case Manager

Responsibilities

• Provide intensive case management and housing stabilization services to homeless Veteran families
• Coordinate client intake, conduct initial assessments, and maintain electronic client files
• Perform street and venue-based outreach to homeless Veterans
• Develop individual budgets and service plans, involving clients in the design of their own supportive services
• Provide service coordination and linkage to VA services, mental health, substance abuse treatment, and vocational assistance
• Conduct home visits and weekly meetings to identify short and long-term goals
• Maintain accurate HMIS database records and prepare programmatic reports
• Collaborate with Housing Specialists to maintain positive relationships with landlords
• Provide crisis intervention as needed to ensure client safety and stability

Requirements

• High school diploma required (Bachelor’s degree in Social Services or related field preferred)
• 1 year of direct support experience with individuals experiencing homelessness, mental health, or substance abuse issues
• Valid California driver license and a reliable personal vehicle
• Proficiency in Microsoft Office suite (Word, Excel, Outlook, and Teams)
• Ability to maintain a calm demeanor in stressful situations and provide excellent customer service
• Ability to pass criminal records clearance

Preferred Qualifications

• Experience with the Homeless Management Information System (HMIS)
• Ability to complete CPR/First Aid training
