About the role
Responsibilities
- Provide intensive housing and vocational case management to homeless Veteran families
- Coordinate the SSVF client intake process, including initial assessments and eligibility determination
- Conduct street and venue-based outreach to homeless Veterans
- Develop individual budgets and service plans to address housing barriers and increase stability
- Provide service coordination and linkage to VA services, mental health, substance abuse treatment, and employment assistance
- Maintain comprehensive electronic case files and update HMIS database records within 24 hours of service delivery
- Conduct home visits and weekly meetings with participants to identify short and long-term goals
- Collaborate with Housing Specialists to maintain positive relationships with landlords
Requirements
- High school diploma or equivalent (Bachelor’s degree in Social Services preferred)
- 1 year of experience providing direct support services to individuals experiencing homelessness, mental health, or substance abuse issues
- Valid California driver license and a reliable personal vehicle
- Proficiency in Microsoft Office suite (Word, Excel, Outlook, and Teams)
- Ability to maintain a calm demeanor and provide effective crisis intervention in stressful situations
- Ability to pass a criminal records clearance
Preferred Qualifications
- Experience using the Homeless Management Information System (HMIS)
- Completion of CPR/First Aid training
