Case Manager

Responsibilities

• Provide intensive housing and vocational case management to homeless Veteran families
• Coordinate the SSVF client intake process, including initial assessments and eligibility determination
• Conduct street and venue-based outreach to homeless Veterans
• Develop individual budgets and service plans to address housing barriers and increase stability
• Provide service coordination and linkage to VA services, mental health, substance abuse treatment, and employment assistance
• Maintain comprehensive electronic case files and update HMIS database records within 24 hours of service delivery
• Conduct home visits and weekly meetings with participants to identify short and long-term goals
• Collaborate with Housing Specialists to maintain positive relationships with landlords

Requirements

• High school diploma or equivalent (Bachelor’s degree in Social Services preferred)
• 1 year of experience providing direct support services to individuals experiencing homelessness, mental health, or substance abuse issues
• Valid California driver license and a reliable personal vehicle
• Proficiency in Microsoft Office suite (Word, Excel, Outlook, and Teams)
• Ability to maintain a calm demeanor and provide effective crisis intervention in stressful situations
• Ability to pass a criminal records clearance

Preferred Qualifications

• Experience using the Homeless Management Information System (HMIS)
• Completion of CPR/First Aid training
