About the role
Responsibilities
- Support survivors of domestic violence and Permanent Supportive Housing participants through intake, assessment, and personalized care planning.
- Screen prospective participants for eligibility and conduct comprehensive assessments using HMIS, VI-SPDAT, and Housing Retention tools.
- Develop, implement, and monitor individualized case plans to help participants achieve self-sustainability and housing stability.
- Maintain a caseload of up to 25 households, providing regular contact and connecting participants to community resources, financial management tools, and public benefits.
- Perform frequent home visits and provide post-placement support to document housing status at various intervals.
- Ensure accurate and timely data entry into the HMIS system, maintaining all case files in compliance with agency and confidentiality standards.
- Participate in internal and external case coordination meetings and stay current on best practices in social services and housing.
Requirements
- Bachelor's degree in social services or a related area (an Associate's degree with relevant experience will be considered).
- Experience working with vulnerable populations, including domestic violence survivors.
- Proficient computer skills, including Microsoft Office Suite (Outlook, Word, and Excel).
- Valid California driver license, a reliable personal vehicle, and current personal auto insurance.
- Ability to pass a criminal background check and provide TB test clearance.
- Ability to work flexible hours occasionally to accommodate participant needs.
Preferred Qualifications
- Bilingual skills.
- Experience with trauma-informed care, harm reduction approaches, and motivational interviewing.
- Knowledge of mental health and substance abuse intervention strategies.
