About the role
Responsibilities
- Screen prospective participants by verifying income, documentation, and eligibility.
- Conduct comprehensive assessments (HMIS, VI-SPDAT) to evaluate participant strengths and risks.
- Develop participant-driven care plans and establish long-term stability goals.
- Provide financial coaching, including budgeting, tax filing assistance, and credit repair.
- Link participants to internal programs and external community resources.
- Maintain a caseload of approximately 20 households, facilitating case conferences and acting as a liaison.
- Manage case closures and exits following all agency and adverse action protocols.
- Maintain up-to-date service data in the HMIS system and ensure case files are completed within 24 hours of interaction.
Requirements
- 1+ years of experience working with low-income or homeless populations.
- Proficient in Microsoft Office Suite (Outlook, Word, and Excel).
- Experience working in diverse settings with various stakeholders and personalities.
- Valid California driver license, reliable personal vehicle, and current personal auto insurance.
- Ability to pass a criminal background check and provide TB test clearance.
