About the role
Responsibilities
- Conduct intake and assessments, including screening prospective participants and verifying eligibility.
- Perform comprehensive assessments using HMIS and VI-SPDAT to evaluate participant strengths and risks.
- Develop and monitor participant-driven care plans and establish long-term stability goals.
- Provide financial coaching, including budgeting, tax filing assistance, and credit repair.
- Link participants to internal programs and external community resources.
- Maintain a caseload of approximately 20 households, facilitating case conferences and acting as a liaison.
- Manage case documentation and data entry in the HMIS system within 24 hours of service interaction.
- Lead workshops on topics such as resume building and financial literacy.
Requirements
- 1+ years of experience working with low-income or homeless populations.
- Proficient in Microsoft Office Suite (Outlook, Word, and Excel).
- Valid California driver license, reliable personal vehicle, and current personal auto insurance.
- Ability to pass a successful criminal background check.
- Ability to provide TB test clearance.
