Case Manager I

Responsibilities

• Conduct intake and assessments, including screening prospective participants and verifying eligibility.
• Perform comprehensive assessments using HMIS and VI-SPDAT to evaluate participant strengths and risks.
• Develop and monitor participant-driven care plans and establish long-term stability goals.
• Provide financial coaching, including budgeting, tax filing assistance, and credit repair.
• Link participants to internal programs and external community resources.
• Maintain a caseload of approximately 20 households, facilitating case conferences and acting as a liaison.
• Manage case documentation and data entry in the HMIS system within 24 hours of service interaction.
• Lead workshops on topics such as resume building and financial literacy.

Requirements

• 1+ years of experience working with low-income or homeless populations.
• Proficient in Microsoft Office Suite (Outlook, Word, and Excel).
• Valid California driver license, reliable personal vehicle, and current personal auto insurance.
• Ability to pass a successful criminal background check.
• Ability to provide TB test clearance.

Preferred Qualifications

• Associate's or Bachelor's degree.
