Case Manager

Responsibilities

• Provide individual and group treatment services to promote client stability and recovery
• Formulate measurable, achievable goals and objectives based on life domains data
• Monitor individual risk to self, others, and property, referring to appropriate services when necessary
• Advocate for individuals to coordinate care internally and externally with relevant services
• Utilize crisis intervention techniques for individual and community stabilization
• Educate individuals on medication needs, side effects, and identifying illness warning signs
• Maintain accurate documentation in client records according to WAC, RCW, and Agency requirements
• Assist with effective discharge planning and treatment closure

Requirements

• Bachelor’s Degree in Psychology, Social Work, or a related field
• Washington State Registered Agency Affiliated Counselor Credential
• Demonstrated assessment skills (risk, suicidal, crisis intervention, and diagnostic) and counseling skills
• Valid Washington State Driver’s License and a safe, reliable vehicle with proof of insurance
• Ability to respond to aggressive behaviors using least restrictive interventions
• Strong written and spoken communication skills
• Basic proficiency in Microsoft Word and Excel

Preferred Qualifications

• Previous experience in case management
• Bilingual or bicultural skills
• Strong attention to detail and accuracy
• Knowledge of County and Agency policies and procedures
