Case Manager

Responsibilities

• Initiate intakes, conduct needs assessments, and develop plans of care
• Provide holistic case management using a psycho-social rehabilitation and recovery model
• Conduct outreach shifts to engage with individuals in the community
• Perform crisis intervention and prevention for clients
• Link clients with community supports including housing, transportation, and legal services
• Advocate for homeless individuals to access essential services
• Document client activities accurately in the electronic medical record (Telus PS Suite)
• Participate in regular supervision, case reviews, and team meetings

Requirements

• Completion of a 2-year post-secondary program (Social Service Worker or Mental Health and Addictions Worker diploma)
• At least two years of experience working with street-involved populations
• Experience in case management with adults and youth facing serious mental health and substance use issues
• Knowledge of recovery and harm reduction-based approaches
• Valid driver's license and access to a reliable vehicle for travel
• Ability to work flexible hours including evenings and weekends
• Successful completion of a criminal background check and Vulnerable Sector Screening

Preferred Qualifications

• Graduate certificate in Addictions and Mental Health
