Case Manager

Responsibilities

• Oversee and monitor the progress of individuals on an assigned caseload
• Ensure staff provides services in accordance with person-centered treatment plans
• Develop and implement service plans and personal futures plans
• Coordinate the scheduling of team meetings and inform all members
• Document consumer progress or regression and maintain appropriate records
• Build positive relationships with funding sources and industry providers
• Monitor and supervise the delivery of service plans and staff training
• Participate in discharge planning and serve on assigned committees

Requirements

• Bachelor's degree in Social Work or a related field
• Strong organizational skills
• Excellent communication skills
• Ability to travel as required by the position

Preferred Qualifications

• Two years of experience in the field of Intellectual or Developmental Disabilities
