Case Manager

Clear Behavioral Health is dedicated to supporting individuals facing mental health and substance use challenges through compassionate, holistic, and evidence-based care. We provide a full continuum of services, including detox, dual diagnosis treatment, residential and outpatient programs, and specialized mental health services.

Responsibilities

• Oversee counselor documentation in clinical sections of client charts
• Facilitate multi-disciplinary treatment planning and updates
• Conduct individual and group sessions for assigned clients
• Coordinate family and conjoint therapy as specified in treatment plans
• Provide daily clinical progress notes in client charts
• Manage up to 25 client-facing service hours per week
• Contact family members and referral sources weekly
• Participate in clinical team meetings and in-service training

Requirements

• Bachelor's Degree in a mental health related field (preferred) or up to 3 years of behavioral health experience
• Experience working in a residential or outpatient setting
• Valid driver’s license
• Knowledge of addiction and human behavior
• Ability to work independently and collaboratively in a fast-paced environment

• Advanced skills in facilitating individual and group counseling
• Cultural competence and ability to work with diverse backgrounds
