Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
• Manage case assignments, draft service plans, and review case progress regularly
• Advocate for clients, assisting them in making informed decisions regarding clinical status and treatment options
• Collaborate with therapists, clients, and family members to ensure behavioral health needs are met
• Facilitate clients in achieving personal independence and assist in developing daily living skills
• Uphold documentation and billing practices associated with CARF and CMS

Requirements

• High school diploma or equivalent
• NPI number or Ohio Medicaid ID
• Strong interpersonal and communication skills
• Knowledge of behavioral health practices and community resources
• Ability to work independently and as part of a team
• Ability to pass Federal and State background checks

Preferred Qualifications

• College degree in Human Services
• CPR certification
• Minimum of 3 years in behavioral health or a related field
